Massive cuff tear

چکیده
Massive rotator cuff tears are not necessarily synonymous with irreparable tears. Technical and biological challenges to a successful repair include inelastic poorquality tendon tissue, scarring, muscle atrophy, and fatty infiltration. Fatty infiltration of the involved rotator cuff muscles has been identified as an important negative prognostic factor for the outcome after repair of massive rotator cuff tears.

Treatment of Massive Irreparable Rotator Cuff Tear with Arthroscopic Subacromial Bursectomy, Biceps Tenotomy, and Tuberoplasty
Background: The alternative surgery for massive and irreparable shoulder rotator cuff tears in older patients is thedebridement of subacromial bursa, biceps tenotomy, and tuberoplasty (reverse acromioplasty). This study aimed toreport the effectiveness of such a treatment performed arthroscopically in a small group of patients for a short periodof time. Methods: This pros...
متن کاملSuperior Capsular Reconstruction With Superimposition of Rotator Cuff Repair for Massive Rotator Cuff Tear
Massive rotator cuff tears are particularly challenging to treat and severely limit the function of the shoulder. Compared with a small rotator cuff tear, massive tears are more unpredictable and usually present with low tendon quality. When performing an anatomical repair of the rotator cuff, the surgical treatment is often associated with failure of the construct. An alternative procedure tha...
متن کاملArthroscopic Repair of Massive Subscapularis and Supraspinatus Tear by Double-Row Knotless Technique
Subscapularis is the most powerful muscle in the rotator cuff. A steep learning curve and constrained space anteriorly make the repair difficult. In massive tears of the rotator cuff, repair of the subscapularis initially makes the massive supraspinatus tear repairable. We present our technique to repair subscapularis and supraspinatus tear by double-row anchors in a knotless and tension-free m...
متن کاملOperative Technique: Arthroscopic Repair of Massive Rotator Cuff Tears
Introduction Rotator cuff tears are a common cause of pain and disability. 2 With modern advances in arthroscopy and arthroscopic techniques, most rotator cuff tears can now be repaired arthroscopically, with several authors reporting successful results. However, the optimal management of massive rotator cuff tears is controversial and evolving. In general, a massive rotator cuff tear (MRCT) is...
متن کاملIncreasing age and tear size reduce rotator cuff repair healing rate at 1 year
Background and purpose - There is a need to understand the reasons why a high proportion of rotator cuff repairs fail to heal. Using data from a large randomized clinical trial, we evaluated age and tear size as risk factors for failure of rotator cuff repair. Patients and methods - Between 2007 and 2014, 65 surgeons from 47 hospitals in the National Health Service (NHS) recruited 447 patients ...
متن کاملPreliminary Results of a Consecutive Series of Large & Massive Rotator Cuff Tears Treated with Arthroscopic Rotator Cuff Repairs Augmented with Extracellular Matrix
Background: Recurrence rate of rotator cuff tears is still high despite the improvements of surgical techniques, materials used and a better knowledge of the healing process of the rotator cuff tendons. Large to massive rotator cuff tears are particularly associated with a high failure rate, especially in elderly. Augmentation of rotator cuff repairs with extracellular matrix or synthetic patc...
